CHECKLIST OF ELIGIBLITY REQUIREMENTS

  • DTI business name registration/SEC registration certificate, whichever is appropriate under laws of the Philippines;
  • Valid and current Mayor's permit/municipal license; (principal place of business)
  • BIR Registration certification, wich contains Taxpayer's Identification Number;
  • BIR Value Added Tax registration;
  • Certification that the applicant is not "blacklisted" or banned from bidding by the government or any of its agencies,
  •       offices, corporation or LGUs, and other private corporations or electric cooperatives; including non-inclusion in the
  •       Consolidated Blacklisting Report issued by the Government Procurement Policy Board (GPPB), as provided in
  •       Section 69.4 of the IRR-A No. 9184.
  • Compliance with E.O.#398
  •       a.Proof of VAT payments for the past six months.
  •       b.Tax Clearance from the BIR to prove applicant's full and timely payment of taxes to the government for last 2 quarters.
  •       c. A Certification under oath  from the suppliers/contractors responsible officers that the supplier/contractor is free and clear of
  •          all liabilities with the government 
  • Copy of latest income tax returns duly stamped and received by BIR and duly validated with tax payments.
  • Statement in matrix form of all ongoing and completed government and private contracts (service contracts, maintenance contracts,
  •        purchase orders, job orders, etc.) within the relevant period, where applicable , including contracts awarded but not yet started,
  •        if any.
  •       The statement shall state whether each contract is:
  •       a.Ongoing, Completed or Awarded but not yet started; within the relevant period, where applicable.
  •          Each contract should include the following:
  •             i. The name of the contract;
  •             ii. Date of contract;
  •             iii. Amount of contract and value of outstanding contracts;
  •             iv. Date of delivery;
  •             v. End-user's acceptance, if completed
  • Manufacturers ISO 9001 Certification/Any Applicable Standard for particulars product to be procured.
  • Manufacturers Certificate or Distributor Certificate
  • Duly signed letter, authorizing the Bac or its duly authorized representatives to verify any or all of the documents submitted for
  •       elegibilty check.
